This policy explains how Carolyn’s Family Clinic ("we," "us") collects, uses, and shares information when you visit https://carolynsfamilyclinic.com and related online pages (the "Site").
Your health information is covered separately. When you use our patient portal, request an appointment, complete intake forms, or otherwise become or act as a patient, the information you give us is protected health information under HIPAA and Texas law. That information is governed by our Notice of Privacy Practices, not this policy. If the two conflict for health information, the Notice of Privacy Practices controls.
Please do not send health details through general contact forms, email, or chat on the public Site. For medical questions, call us at 469-795-3311 or use the secure patient portal. In an emergency, call 911.
Information we collect
Information you give us. When you fill out a general contact form, sign up for our newsletter, apply for a job, or leave feedback, we collect what you enter — such as your name, email, phone number, and message.
Information collected automatically. When you browse the Site, we and our service providers collect technical data, including:
- IP address and approximate location (city or region)

- Pages viewed, links clicked, time on page, and the site that referred you
- Date and time of your visit
Information from others. We may receive information from online review platforms, social media pages you interact with, or referral sources.
We do not collect credit scores, and we do not knowingly collect Social Security numbers, insurance details, or medical information through the public Site.

Security and retention
We use reasonable administrative, technical, and physical safeguards, including encrypted (HTTPS) connections. No website is completely secure, so please avoid sending sensitive information through general forms or email. If a breach affects your personal information, we will notify you as required by Texas law (Business & Commerce Code Chapter 521). We keep website information only as long as needed for the purposes above or as the law requires.
Children
The Site is not directed to children under 13, and we do not knowingly collect their personal information online. Parents seeking care for a child should call us or use the patient portal. If you believe a child has sent us information, contact us and we will delete it.
Links to other sites
