Carolyn’s Family Clinic offers telehealth services that allow patients to consult with a licensed healthcare provider using secure video for synchronous communication. By using these services, you acknowledge and agree to the following:
Not for emergencies. Telehealth is not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
Limitations of remote care. Telehealth visits do not include an in-person physical examination. Your provider may be unable to fully assess some conditions remotely and may recommend an in-person visit, testing, or referral to another provider.
Technology risks. Technical problems such as poor connection, equipment failure, or low video or audio quality may interrupt or delay your visit or limit what your provider can evaluate. Carolyn’s Family Clinic uses reasonable safeguards but cannot guarantee uninterrupted service.
Privacy and security. We use HIPAA-compliant platforms and take reasonable steps to protect your health information. However, no electronic transmission is completely secure. Please join from a private location and use a secure internet connection whenever possible.
Location and licensure. Providers can only treat patients located in states where they are licensed. You must be physically located in Texas at the time of your visit, and you agree to confirm your location when asked.
Prescriptions. Prescribing is at the provider's professional discretion and subject to state and federal law. Some medications, including certain controlled substances, may not be prescribed through telehealth or by your provider.
Voluntary participation. Telehealth is voluntary. You may decline or stop a telehealth visit at any time and request in-person care without affecting your right to future treatment.
Billing. Telehealth visits may be billed to your insurance or to you directly. Coverage varies by plan, so please check with your insurer about telehealth benefits.
By scheduling or taking part in a telehealth visit, you confirm that you have read and understood this disclaimer and consent to receive care through telehealth.
